Scheduling and Payment

Step 1: Reserve your spot

To reserve an assessment appointment, families will place a $175 nonrefundable reservation deposit. This deposit is applied toward the total assessment fee. The total cost of this assessment is $2,463.

Step 2: Enrollment and payment set-up

After your spot is reserved, LifeFx will reach out to:

• • Share enrollment paperwork
  • Confirm assessment details
  • Send your invoice and help you choose the best payment option for your family
    

Step 3: Choose your payment option

Families may choose one of the following:

• Option A: Payment plan
  Families may choose a payment plan and select payment dates in coordination with LifeFx, as long as at least 50% is paid 7 days prior to the assessment and the remaining balance is paid before the feedback meeting is held and final written report is released.
• Option B: Pay in full
  Families may also choose to pay the full assessment fee, payment must be paid at least 7 days prior to the assessment date.

Step 4: Important deadline

To keep scheduling fair and ensure we can offer openings to other families, payment deadlines are firm. If the required payment is not received 7 days prior to the assessment date, the appointment hold will be automatically released and the reserved spot will be forfeited. Reminder emails will be sent 72 hours and 24 hours before the deadline.

Deposit policy and rescheduling

The $175 reservation deposit is nonrefundable. If you need to reschedule, the deposit can be:

• Applied to a new assessment date right away, or
• Held for up to 12 months and applied to a future assessment appointment
